This month, we’re spotlighting our recent LSIT winners whose keen observations and questioning attitudes have prevented potential incidents and reinforced our safety culture.
Grand Prize Winner: Kwang Ye “Miss Kim” Conrad, LSIT SLOTHS
During a critical pre-entry inspection, Miss Kim, fully dressed in PAPRs, meticulously reviewed the equipment. Her sharp eye caught a small, yet significant, crack in a buddy’s PAPR hose. Without hesitation, she promptly retrieved a replacement hose and assisted with its proper installation. Her diligent questioning and meticulous attention to detail ensured her colleague could safely enter Alpha-4, averting a potentially serious situation.
LSIT Winner: Authur Gray, BOBCATS
Authur’s insightful questioning brought to light a crucial safety concern regarding the operational limits of bubble suits. He inquired about the minimum temperature required for bubble suits to function effectively. It was confirmed that the lowest safe operating temperature for these suits is 40 degrees. Given that the ambient temperature was well below this threshold, Authur’s proactive question allowed for the issue to be addressed and mitigated before entry, ensuring operations remained within the bubble suit’s specified limits of use.
LSIT Winner: Karl Klinger, T-2 DOBES
Following an emergency evacuation drill on October 28, 2025, in 1916-T2, Karl observed difficulties in locating Local Emergency Supervisors (LES) for negative accountability reporting. To improve future emergency response efficiency, Karl suggested that LES personnel enhance their visibility during drills and actual events. His valuable feedback aims to ensure easier identification and location of LES, thereby streamlining the negative accountability process for their respective zones.
